Allergy and Asthma Network Mothers of Asthmatics (AANMA) is a leading non-profit organization dedicated to eliminating suffering and death due to asthma, allergies, and related conditions. Founded in 1985, AANMA’s core areas of expertise are education, advocacy, and outreach. AANMA’s flagship publications, “Allergy & Asthma Today” magazine and “The MA Report” newsletter and website are consumer lifelines to medical news and healthy living.

Alzheimers Aid Society of Northern California

Alzheimer’s Aid Society of Northern California provides families with education, support, guidance, and referral services. The Society promotes education to the general public and the professional community and conducts in-services to skilled nursing, residential facilities, and law enforcement. The Society also contributes to research seeking the cause, possible prevention, treatment and cure of Alzheimers disease. The Society works to educate legislators about Alzheimers disease, as well as support California legislation in the areas of increased funding for research, long-term insurance for patients, and respite for caregivers. A 52-page booklet entitled, “A Practical Guide for Alzheimer Caregivers” is made available to all families and caregivers who call. Additionally, scholarships are awarded to college students studying the field of gerontology.

The Society also features support groups whereby individuals receive support and fellowship from others experiencing similar feelings and emotions. The group is designed to provide support as well as information on research, therapy and other topics of interest. Weekly support groups for both the caregiver and the patient are held at Oasis of Stockton, 1119 Rosemarie Lane. Monthly support group meetings for caregivers are held at Somerford Place, 3530 Deer Park Drive in Stockton. There are also groups in Sacramento, Tracy, and Turlock.

Alzheimers Association - Greater Sacramento Office

Alzheimers Association - Greater Sacramento Offices mission is to eliminate Alzheimers through the advancement of research, to provide enhanced care and support to all affected, and to reduce the risk of dementia through the promotion of brain health.

The office serves eight counties in the Greater Sacramento area, including San Joaquin. Over 60,000 people in the region have Alzheimer’s disease or another form of dementia and over 180,000 caregivers provide support.

The free local programs and services include a 24-hour per day, 7 days per week helpline, care consultations for families, and over 20 support groups. Caregiver education forums and community education classes related to dementia and brain health are also offered.

American Cancer Society

American Cancer Society provides support to cancer patients and their families at no direct cost to patients or the people who care for them. This includes transportation to medical appointments, transportation reimbursement, lodging, support groups and wigs.

American Cancer Society also provides information on all types of cancer and prevention education. A live chat line is available.

Cancer Support Groups hosts two cancer support groups:

  • "Reach to Recovery" puts women with breast cancer into contact with other breast cancer survivors.

American Council of the Blind

  • Family members, friends, doctors, and others who care for, work with, and are trying to assist a blind or visually impaired person.

American Council of the Blind (ACB) is a national organization of blind, visually impaired, and sighted individuals whose purpose is to work toward independence, security, equality of opportunity, and improved quality of life for all blind and visually impaired people. ACB programs and services include:

  • "ACB Braille Forum," a bimonthly publication available in braille, large print, on NLS-style cartridge (audio), by email subscription, podcast, and at www.acb.org. It also plays on ACB Media Networks Channel 10. The magazine is free to members; all others pay $25 per format per year.
  • A number of college scholarships (must be legally blind in both eyes). The application goes live on acb.org on November 1, and must be returned by the deadline.
  • An annual conference featuring a large trade show of the latest high- and low-tech gadgets for blind and visually impaired individuals, as well as a wide variety of breakout sessions, seminars, workshops, technology training classes, and more. Volunteer opportunities are available; contact the national office for details.
  • An internet radio station, ACB Media Network (https://acbmedia.org/), with a variety of streams. ACB Radio Café features music by blind musicians. Mainstream features include content from the International Association of Audio Information Services as well as a variety of talk shows. Its mission is to prevent and cure diabetes to help improve the lives of all people affected by diabetes. Information and literature about diabetes, nutrition, and the complications of the disease are available on an as-needed basis by contacting the office.

American Heart Association (AHA)/American Stroke Association is a nonprofit voluntary health agency funded by private contributions. The AHA are dedicated to the reduction of death and disability from cardiovascular diseases like heart diseases and stroke.

The association is one of the worlds largest voluntary health organizations with about 2,200 community organizations in all states, the District of Columbia, and Puerto Rico. More than 3.7 million people volunteer with the American Heart Association to fight cardiovascular diseases, the nations number one killer and a leading cause of disability. Preventing heart disease and stroke is the first priority of the American Heart Association. American Lung Association in California

American Lung Association in California works to prevent lung disease and promote lung health through education, community service, advocacy and research. Lung disease, including asthma, emphysema and lung cancer, is the third leading cause of death in America.

Please call the American Lung Association for answers to questions about asthma, chronic obstructive pulmonary disease (C.O.P.D.), emphysema, tuberculosis and other lung diseases as well as questions about the health effects of air pollution, the harmful effects of tobacco, the prevalence of youth smoking and other concerns related to lung health. Catholic Charities of the Diocese of Stockton has provided social services to people in San Joaquin, Stanislaus, Calaveras, Tuolumne, Alpine, and Mono counties, for over 80 years; and it has served as a sign of hope in local communities for people of all ages, religions, and races.

Today Catholic Charities provides over twenty-four vital programs and services to the most vulnerable within our communities.

Catholic Charities of the Diocese of Stockton (Health Care Access Program)

Health Care Access Program assists uninsured individuals in applying for Medi-Cal, Medi-Cal Expansion and Kaiser Child Health Plan. Certified Enrollment Counselors (CECs) provide case management to clients to ensure utilization and retention. The program also assists with annual renewals and selection of a doctor or a dentist.

CECs help guide individuals with navigating the health care coverage system in both Stanislaus and San Joaquin Counties and are bilingual in Spanish/English. CECs provide outreach/presentations in the community and are knowledgeable about the Affordable Care Act.

Another service includes Healthy Smiles- connecting young children with Medi-Cal Dental services, assisting with identifying and selecting a dental home, and scheduling a dental appointment.

CDA Foundation (CDA Cares)

Californians in need of dental care, including anyone having difficulty accessing dental care, uninsured, underinsured, unemployed, and adults and children of any age.

CDA Cares is to improve the oral health of all Californians by supporting the dental profession in its efforts to meet community needs. Oral health is critical to the total health and well-being of an individual. For young children, it means they won’t live with pain or miss school. For adults, it can enhance employment outcomes, improve their overall health and quality of life and, for many, restore dignity.

CDA Cares facilitates events and grant opportunities to community-based organizations and individuals who reduce barriers to care in underserved populations. The dental profession and its philanthropic values is represented as it generously gives back to help meet the needs of Californians. CDA Cares offers extractions, fillings, cleanings, oral health education and assistance finding a source for ongoing dental care. On a limited basis, CDA Cares also offers root canals, dentures and temporary partial dentures.

Help Me Grow Call Center provides developmental and social-emotional screenings of children from birth to 5 years old, makes referrals based on the childs needs and conducts follow-ups. Uses the Ages and Stages Questionnaire (ASQ), a nationally recognized model. Services provided by a Community Resource Specialist.

Family Resource Center (San Joaquin Treatment & Education for Everyone on Teeth and Health (SJ TEETH))

LFCE is also affiliated and involved with People and Congregations Together (PACT) and Pacific Institute for Organizing (PICO) to promote, motivate and organize the Hmong and Laotian community in developing leadership, organization and advocacy skills through one-on-one appointments and community action meetings.

LFCE offers:

  • The Home Visitation Project - a collaborative effort among Lao Family, UC Davis Cooperative Extension Program, SUSD schools and LUSD schools. The program offers home-based health and nutrition education, a Raising a Reader family literacy program, parent workshops, and health screening for children and to parents or guardians of children ages 0 to 5 years living within the designated zip codes.
  • Mental Health Programs - Southeast Asian Recovery Services (SEARS) provides case management to mental health consumers and also conducts outreach and engagement to the Southeast Asian communities.

National Alliance on Mental Illness of San Joaquin County (NAMI SJC) provides personal and mutual support for families and individuals living with a mental illness, including information and referral services. It assists families and individuals with coping and survival skills.

NAMI currently offers:

  • The Family-to-Family class is an eight-week course for families and friends of someone living with mental illness. The course covers mental illness, medication, coping skills, and family emotional support. This course is also available in Spanish. At this time, it is being offered virtually.   • Peer-to-Peer class is an eight-week education course for consumers of mental health in the community. In addition, IOOV “In Our Own Voice” presentations are available for any group wishing to hear from two individuals who are in recovery from mental illness. This course is also available in Spanish.
  • The NAMI Provider Education Program offers 12-1/2 hours of in-service training, taught by a trained five-member team of family members, individuals living with mental illness, and a mental health professional.
  • The NAMI Basics is a six-week, peer-directed education program developed specifically for parents and other family caregivers of children and adolescents who have been diagnosed with a serious mental illness/serious emotional disturbance or who are experiencing symptoms but have not yet been diagnosed.
  • The NAMI Connection Recovery Support Groups meet for 90 minutes and is open to all adults with mental health challenges, with or without a diagnosis. NEDA supports Eating Disorders Awareness Week (EDAW) held during the last week of February each year. There are over 350 EDAW coordinators, teachers, social workers, eating disorder professionals and college teams who distribute educational materials and organize awareness and prevention events on high school and college campuses, in hospitals and community centers in all 50 states.

Bristol Hospice-Stockton works together with patients and their families to enhance quality of life. Hospice services are available for those typically affected by any type of life-limiting condition. By utilizing the most modern pain and symptom management techniques combined with a compassionate support system, hospice services help to reduce the overwhelming stress and anxiety that can come with caring for a loved one with a life-limiting illness at home or in a residency facility.

Hospice Care Team: When the focus of medical care changes from cure to comfort, the Hospice Care team provides physical, emotional, practical, and spiritual support for the entire family. Hospice provides home visits by nursing staff, medical social services, therapy services, physician services, hospice aide, and volunteer services.

Bereavement/Spiritual Counseling: Hospice also includes individual or group counseling. The professional team can provide individual counseling as well as offer regularly scheduled community groups. Hospice has developed the "Kids Grief Connection" for children experiencing the loss of a friend or a loved one, including a family member. All services are provided to the community free of charge.

Rite of Passage California Foster Families is a private non-profit foster family agency licensed by the State of California to recruit, train and provide extensive services to foster/resource family homes. The goal is provide safe, ethical and loving homes for foster youth ages infant to 17 years. 24-hours per day, 7 days per week emergency support along with weekly home visits by a social worker with a masters degree are provided. In-home training for families is offered and reimbursement is given to all homes with children.

Sudden Infant Death Syndrome (SIDS) program provides grief and bereavement services to families and/or child care providers who have experienced the death of an infant due to SIDS or any Sudden Unexpected Infant Death (SUID). A Public Health Nurse will make contact with the family within three working days after notification from the coroner about the death, and provide supportive services, information, and referrals to community resources.

Community and provider education about SIDS are offered anytime.

St. Josephs Cancer Institute (SJCI) is the only accredited cancer program in San Joaquin County. SJCI includes a dedicated inpatient oncology department and state-of-the-art radiation therapy.

In addition, St. Joseph’s provides cancer support groups and support for patients and families dealing with cancer. This includes a 6-week cancer survivng and thriving class, free medically-supervised exercise program to assist with fighting fatigue, gaining stamina, and more.

The Navigator Program is available to help anyone with cancer navigate through the journey of cancer from just receiving the news that the biopsy was positive, through the tests, procedures, treatment, follow-up, and long-term survivorship. St. Josephs Medical Center (Power Hour plus Sugar Fix)

Power Hour Empowering individuals with knowledge and support is the goal of this no cost program available to any San Joaquin County resident. Participants will gain knowledge, skills, and confidence from a Certified Diabetes Care and Education Specialist (CDCES) to improve health outcomes and quality of life

Sugar Fix program offers support to help individuals keep their glucose levels in a healthy range. Please call to register.

Join the Sugar Fix group for informative presentations and support through the highs and lows of your journey with diabetes. Sessions are free and open to all; caregivers and family members are welcome.

Stellar Career College (Dental Assisting, Medical Assisting, Veterinary Assisting, Medical Office, Accounting Certificate Programs in Modesto)

Eligibility:

Anyone ages 18 years or older. Must have High School Diploma or equivalent.

Fees: Vary; Financial aid available to those who qualify.

Stellar Career College is to provide consistent high-quality instruction and motivation in a positive learning environment. The welfare and education of students and employees at their technical school is their primary focus. The focus is on building skilled individuals and a successful trade school to serve the needs of the community.

Sutter Tracy Community Hospital (Breastfeeding Education and Supplies)

Eligibility:
  • Expectant and new mothers
  • Open to the community
  • By appointment only

Breastfeeding education, lactation support, and breastfeeding supplies are available by appointment. Certified Lactation Consultants offer support to expecting and new moms needing information and support for successful breastfeeding of their child.

Sutter Tracy Community Hospital (Childbirth Education Classes)

Eligibility:

Expectant parents; pre-registration is required. Currently only offered for those delivering at Sutter Tracy Community Hospital.

Fees: None

Childbirth Education Classes draw upon a variety of techniques. Childbirth instructor prepares expectant mothers and their partners for birth. The class includes instruction in relaxation and breathing to reduce the discomfort of labor, the partners role, what to expect upon admission to the department, possible medical procedures, medicated and non-medicated labor and anesthesia, postpartum and newborn care.

Tours of the Obstetric department are not yet being offered at this time.

The Organizing and Leadership Academy

Eligibility:

The Organizing & Leadership Academy (TOLA) is a rigorous “hands-on” training program for individuals interested and motivated to pursue a career in organizing. The TOLA program is demanding and effective.

TOLA Fellows participate in classroom instruction, do significant reading and reflective writing, and are challenged by experienced organizers, community leaders and elected officials. Most importantly, fellows work on real projects to develop the discipline and skills necessary to become effective organizers.

University of California Cooperative Extension of San Joaquin County (Expanded Food and Nutrition Education Program (EFNEP))

Eligibility:

Participants must satisfy one of the following:

  • Receive or be eligible to receive federal food assistance
  • Have a monthly household cash income that is at or below 185% of the Federal Poverty Level
  • Be a pregnant teen or foster youth in transition, regardless of income or age
  • Senior or guardian or cares for young children
  • Highschool Student

Expanded Food and Nutrition Education Program (EFNEP) participants learn to:

  • Plan nutritious meals
  • Increase physical activity
  • Stretch their food dollar
  • Practice safe food handling
  • Prevent obesity through healthy lifestyles
  • Practice life skills

Adult Individuals, families with children under 18 years of age. Participants should be responsible for planning and preparing family meals.

Youth K-12 student attending a San Joaquin County eligible school (participates in free a reduced priced lunch program etc.)

University of California Cooperative Extension of San Joaquin County (Healthy San Joaquin Collaborative)

Must work in or reside in San Joaquin County.

Healthy San Joaquin Collaborative is a member organization of San Joaquin County that works collectively to bring about individual, community, system and environmental changes to shape nutrition and physical activity behaviors.

University of California Cooperative Extension of San Joaquin County (CalFresh Healthy Living, UC Program )

Eligibility:

Low-income families in San Joaquin County.

Fees: None

CalFresh Healthy Living, UC Program offers workshops at local agencies serving low-income families and individuals in San Joaquin County. The workshops promote healthy eating, physical activity, food resource management skills and safe food handling. The following is a list of their programs:

  • Youth program offers San Joaquin County children research-based nutrition and physical activity focused curicula. The information helps them make consistent, healthful choices in their daily lives. Audience: children, birth to 18 years of age enrolled in programs where there is 50% or more enrollment in free and reduced price lunch program or living in census tracts where 50% or more of the residents are below 185% of the Federal Poverty Level
  • Adult program is focused on improving the nutrition-related skills, specifically those skills related to selecting, purchasing and preparing a low-cost nutritional diet for themselves and their families

Wellness WORKs! (Work Wellness: The Basics)

Eligibility:

San Joaquin County CalWORKs participants. Must be referred by a CalWORKs Employment and Training Specialist (case manager).

Wellness WORKs! is a center that provides wellness classes and activities to the San Joaquin County CalWORKs population. The Wellness WORKs! program teaches participants the tools needed to become a healthier and productive person in both the home and the workplace.

The One Week program may be used for CalWORKs participants that have been identified by their case managers as having the following barriers or needs:

  • Frequent employment losses (firing or quits)
  • Prerequisite for participants without a job history before they enter supervised job search through the County
  • Participants with secured employment but position will not start for one or more weeks
  • Participants in need of life skills training as identified by a career assessment counselor
  • Having difficulties making satisfactory progress or following through on assigned activities
  • Participants who are waiting to start vocational education, a job training program, or during education breaks

Work Wellness: The Basics is a One Week program which covers:

  • Success is a Choice
  • Dress for Success and Image Building
  • Temperaments in the Work Place
  • Health Issues that Interfere with Job Performance and Attendance
  • Physical Wellness
  • Healthy Eating for Better Job Performance
  • The Working Parent
  • Conflict Resolution in the Work Place
  • Managing Job Stress
  • Safety and Prevention of Personal and Workplace Violence

The Three Week program is recommended for participants with one or more serious barriers to successfully transitioning to the workforce. Examples of participants who may qualify for the extended curriculum are those recovering from drugs and alcohol, participants with serious health problems (mental, emotional, or physical), those in an abusive relationship or suffering from low self-esteem, and those with minimal life skills. The topics include:

  • Anger Management
  • Drugs and Alcohol in the Workplace (not currently offered on zoom)
  • Money Management

Wellness WORKs! also offers a One Day program that includes a two-year certification through EMS Safety Services in the following areas:

  • Basic CPR and First Aid
  • CPR/AED for Professional Rescuers
  • Child care CPR/First Aid
