Services
CARE LINK "Health Care for the Homeless" provides access to primary health care for the greater Stockton area's homeless population. CARE LINK does the following:
Provides primary health care services for the homeless at the Gleason House Medical Clinic, 423 South San Joaquin Street, Stockton, all Community Medical Centers, Inc., clinic sites, and the Care Link Outreach Program at locations throughout San Joaquin County
Provides medical outreach services and street outreach to inform homeless individuals of the available services, including entitlement programs
Provides case management of primary health care needs
Refers homeless persons for needed specialty care services
Refers homeless persons for needed mental health and substance abuse services
Assists homeless individuals in establishing eligibility for housing and entitlement programs
