• Those who wish to live somewhere other than with family or a board and care facility.

Services

California MENTOR is a Sevita Health company established in 1980 that provides community-based services for adults with intellectual and developmental disabilities. MENTOR offers specialized residential, behavioral, and, most notably, Family Home Agency (FHA) services, where individuals live with mentors in private homes. 

Currently, only one to two clients are accepted per home. Case management services are provided. Bi-weekly home visits with the program coordinators are made.

Monday through Friday 8:30 a.m. to 5:00 p.m.
