• Medi-Cal eligible children/youth under 21 years of age (includes undocumented children)
• Non-Medi-Cal eligible children/youth under 19 years of age with family incomes at or below 200 percent of the federal income guidelines (includes undocumented children)
• Resident of San Joaquin County
• Children in Foster Care

Child Health and Disability Prevention Program contracts with medical providers who provide complete health assessments for the early detection and prevention of the diseases and disabilities in children and youth. The program staff members work collaboratively with health care providers to ensure that eligible children receive quality health care. They also assist families with finding a primary care physician and dentist.

A complete health assessment includes:

• Growth and development check

• All needed shots and lab tests

• Dental

• Hearing screening

• Vision

• Appropriate health education information
