Open to all San Joaquin County residents in need of health care.
Health Care Access Program assists uninsured individuals in applying for Medi-Cal, Medi-Cal Expansion and Kaiser Child Health Plan. Certified Enrollment Counselors (CECs) provide case management to clients to ensure utilization and retention. The program also assists with annual renewals and selection of a doctor or a dentist.
CECs help guide individuals with navigating the health care coverage system in both Stanislaus and San Joaquin Counties and are bilingual in Spanish/English. CECs provide outreach/presentations in the community and are knowledgeable about the Affordable Care Act.
Another service includes Healthy Smiles- connecting young children with Medi-Cal Dental services, assisting with identifying and selecting a dental home, and scheduling a dental appointment.
