Stockton PACE is an integrated health plan and care team that provides medical and social services to keep the frailest elderly living in the community. An interdisciplinary team of doctors, nurses, social workers, rehab specialists, and aides are provided to meet the participants needs in the facility and in the community.

Eligibility includes all of the following:

• Be age 55 or older
• Reside in San Joaquin or Stanislaus counties (zip codes at stocktonpace.welbehealth.com/about/)
• Be certified by the State as eligible for nursing facility level of care
• Be able to live safely at home and in the community without jeopardizing the health and safety of themselves and others
