The Sow A Seed Community Foundation Full Circle Assessment Program serves youth in Tracy, California, and surrounding communities who are facing emotional, behavioral, or mental health concerns. The program provides assessments, counseling, and mentoring for individuals with mild to moderate challenges.

The Program provides the following services:

• Individual Counseling
• Case Management
• Assessment
• Peer Groups
• Family Support
• Referral Linkage
• Mentoring
• Treatment Planning
