Pregnant or parenting a child up to one year of age or parenting a child with special healthcare needs up to age 5 who resides in San Joaquin County.
San Joaquin County Public Health Services Nurses Home Visiting program provides education on issues relating to pregnancy and infant development. They also provide assistance in obtaining prenatal care and health insurance coverage.
Home visits are provided to pregnant women, women with high risk pregnancies, high risk infants, postpartum mothers, and for infants and children with special healthcare needs. Nurses provide parent education, case management, developmental screenings, and ensure families are connected to medical and social resources within the community.
