Pregnant or parenting a child up to one year of age or parenting a child with special healthcare needs up to age 3 who resides in San Joaquin County.

San Joaquin County Public Health Services, Nurse Home Visiting program provides education on issues relating to pregnancy and infant development. Assistance is provided in obtaining prenatal care and health insurance coverage.

Home visits are provided to pregnant and postpartum parenting families experiencing high risk pregnancies, high risk infants and for infants and children with special healthcare needs up to age 3. Nurses provide case management, developmental screenings, postpartum depression screenings, parent education on a wide range of topics and ensure families are connected to medical and social resources within the community to meet their needs. 

Monday through Friday 8:00 a.m. to 5:00 p.m.
