Patient Care: terminally ill patients with prognosis of six months or less.

Bereavement Support Groups and workshops are for those who have lost loved ones. Programs are offered to children, teens and adults.

Services

Hospice of San Joaquin believes that everyone deserves a comfortable and peaceful closure. Their professional team offers a comprehensive care plan which includes professional medical, social, spiritual and emotional care (support) for the patient, family and caregivers. Each patient's individualized plan of care is designed to fit specific needs and choices. Visits are as patient and family require or request by the appropriate hospice team member. 24-hour on-call hospice nurse.

Continuous care if a higher level of care is needed. All necessary medical supplies and equipment. Instruction and support in care giving.

Respite care, support and practical help. Counseling and spiritual support, and grief support. Some workshops have a one-time materials fee.

Hospice of San Joaquin offers services regardless of age, including pediatric hospice care. The agency's effective pain control and symptom management are tailored to the individual's changing conditions. Practical day-to-day support and guidance is given to family and caregivers as they adapt to changes in the health of their loves ones.

  • Hospice House, a comfortable "Home" when you need it most - is the first and only residential campus for hospice patients in San Joaquin County. This comfortable house offers an alternative to home based-hospice care when patients are no longer able to remain in their own home.
  • Transitions Program is a community service program designed to provide palliative education, emotional support and community resource information to participants with life-limiting or advancing chronic life-limiting illnesses.
  • The Bereavement Program supports grieving families and friends, including bereavement support groups for both adults and children. Hospice of San Joaquin also provides Community Education on end-of-life issues, and an intensive Volunteer Training course (up to 30 hours) preceding volunteer assignment to Patient Care or Bereavement Team.
