Community Hospice is not a place, but a philosophy of care. They believe that when someone is facing an advanced terminal illness they are not alone. Their team of expert physicians, nurses, hospice aides, social workers and others, step into a patient's home environment to support the patient and family as they navigate their end of life journey. Their goal is to optimize hope, provide comfort, and enhance quality of life, helping patients make decisions about how and where they want to spend the rest of their life.

Palliative Care focuses on enhancing comfort and improving quality of life for individuals facing a serious (not terminal) illness. Palliative Care may be provided at any time during a patient's illness and delivered while seeking curative and life-prolonging treatments.
