Community Hospices team of expert physicians, nurses, hospice aides, social workers and others, step into a patients home environment to support the patient and family as the patient navigates their end of life journey. The goal is to optimize hope, provide comfort, and enhance quality of life, helping patients make decisions about how and where they want to spend the rest of their life. Palliative Care focuses on enhancing comfort and improving quality of life for individuals facing a serious (not terminal) illness. Palliative Care may be provided at any time during a patients illness and delivered while seeking curative and life-prolonging treatments. Hope Counseling offers compassionate and high-quality care using evidence-based practices to meet the needs of people with family, mental health and behavioral issues. Therapeutic support is available for children, teens, individuals, couples and families who may be experiencing issues that have become stumbling blocks in their daily living impeding their ability to live fully.
