The program provides friendly, stimulating care and support through a variety of daily activities that encourage group participation. American Cancer Society provides support to cancer patients and their families at no direct cost to patients or the people who care for them. This includes transportation to medical appointments, transportation reimbursement, lodging, support groups and wigs.

California Poison Control System (CPCS) is the primary source of poison information and treatment advice for all California residents. Health Care Access Program assists uninsured individuals in applying for Medi-Cal, Medi-Cal Expansion and Kaiser Child Health Plan. Certified Enrollment Counselors (CECs) provide case management to clients to ensure utilization and retention. The program also assists with annual renewals and selection of a doctor or a dentist.
CECs help guide individuals with navigating the health care coverage system in both Stanislaus and San Joaquin Counties and are bilingual in Spanish/English. CECs provide outreach/presentations in the community and are knowledgeable about the Affordable Care Act.
Another service includes Healthy Smiles- connecting young children with Medi-Cal Dental services, assisting with identifying and selecting a dental home, and scheduling a dental appointment.
Californians in need of dental care, including anyone having difficulty accessing dental care, uninsured, underinsured, unemployed, and adults and children of any age.
CDA Cares is to improve the oral health of all Californians by supporting the dental profession in its efforts to meet community needs. Oral health is critical to the total health and well-being of an individual. For young children, it means they won’t live with pain or miss school. For adults, it can enhance employment outcomes, improve their overall health and quality of life and, for many, restore dignity.
CDA Cares facilitates events and grant opportunities to community-based organizations and individuals who reduce barriers to care in underserved populations. The dental profession and its philanthropic values is represented as it generously gives back to help meet the needs of Californians. Hospice of San Joaquin believes that everyone deserves a comfortable and peaceful closure. The professional team offers a comprehensive care plan which includes professional medical, social, spiritual and emotional care (support) for the patient, family and caregivers. Each patients individualized plan of care is designed to fit specific needs and choices. Visits are as patient and family require or request by the appropriate hospice team member. A 24-hour on-call hospice nurse are available.
Respite care, support and practical help are offered. Counseling, spiritual support and grief support are also included. Some workshops have a one-time materials fee.

Adult services include crisis intervention and referrals, transitional housing and case management, job skills, resume writing, and outreach. Services for youth and children include counseling, case management, and teen mentoring through the "My Brothers Keeper" program. Medi-Cal Access Program (MCAP) offers low-cost health coverage for mid-income pregnant women. It has been designed for middle income families without health insurance and whose income is too high for no-cost Medi-Cal. one.Charter Elementary is a kindergarten through sixth grade charter school. The school also serves children in transition or homeless children. The schools goal is to satisfy the physical, emotional and educational needs of children in San Joaquin County, using a strong academic program that includes standards-based education with a strong emphasis on literacy and math skills. Children are provided with breakfast, lunch, school supplies, clothing, and other necessities.
Counselors, interns and a school clinician are available to assist children through traumas associated with neglect, abandonment, physical, emotional, and sexual abuse as well as homelessness, hunger, physical and educational issues. By utilizing the most modern pain and symptom management techniques combined with a compassionate support system, hospice services help to reduce the overwhelming stress and anxiety that can come with caring for a loved one with a life-limiting illness at home or in a residency facility.
Hospice Care Team: When the focus of medical care changes from cure to comfort, the Hospice Care team provides physical, emotional, practical, and spiritual support for the entire family. Hospice provides home visits by nursing staff, medical social services, therapy services, physician services, hospice aide, and volunteer services.
Bereavement/Spiritual Counseling: Hospice also includes individual or group counseling. The professional team can provide individual counseling as well as offer regularly scheduled community groups. RiteCare Childhood Language Center of Stockton offers quality speech-language pathology services to children and individuals of the community, utilizing best practices, current research, and individualized client-centered treatment planning. Funding for the clinic facility comes from members of the Scottish Rite organization. There is an approximate eight month to a year wait to be evaluated. After the evaluation, the child will be placed on the waiting list for therapy, if warranted.
The clinicians at the Center are senior and graduate students working on a bachelor or master degree in Speech-Language Pathology at University of the Pacific, who are supervised by certified, licensed speech-language pathologists who are faculty members at University of the Pacific.

Access Managed Care provides telephone linkage to services for individuals in need of mental health and/or substance use treatment. A clinical staff will speak to the caller or caregiver requesting services in order to determine need and eligibility. Upon completion of screening process, individuals can be referred to specialty mental health services, Non specialty mental health services, a substance use treatment program or their identified mental health provider through their health plan.

San Joaquin County In-Home Supportive Services (IHSS) Public Authority (PA) operates a Registry of available home care providers for IHSS recipients.
The IHSS PA offers training classes such as, infection control, fall prevention and dementia care and many others, via an online portal (Care Academy) and certificates are issued once a class is successfully completed.
The IHSS PA is always looking for individuals who are professional, courteous and responsible to work as IHSS providers through the IHSS PA Registry.

Black and African-American pregnant and parenting women, age 16 years and older, and up to six months postpartum.
Black Infant Health is designed to encourage and support a healthy pregnancy, healthy newborn, and women’s health. Twenty group intervention sessions are provided (ten sessions prenatally, ten sessions postpartum), as well as complementary case management.

California Children’s Services (CCS) is a statewide program that arranges, directs, and pays for medical care, equipment and rehabilitation when these services are authorized by the program. CCS pays for treatment, such as doctor services, hospital and surgical care, physical and/or occupational therapy, laboratory tests, X-rays, orthopedic appliances, and medical equipment as needed for the childs CCS-eligible condition.
Child Health and Disability Prevention Program contracts with medical providers who provide complete health assessments for the early detection and prevention of the diseases and disabilities in children and youth. The program staff members work collaboratively with health care providers to ensure that eligible children receive quality health care. Families are assisted with finding a primary care physician and dentist.

Medical Therapy Program (MTP) is a special program within California Children’s Services that provides physical therapy, occupational therapy, and medical therapy conference services in outpatient clinics located in designated public schools for children who have handicapping conditions generally due to neurological or musculoskeletal disorders.

San Joaquin County Public Health Services Nurses Home Visiting program provides education on issues relating to pregnancy and infant development. Assistance is provided in obtaining prenatal care and health insurance coverage.
Home visits are provided to pregnant women, women with high risk pregnancies, high risk infants, postpartum mothers, and for infants and children with special healthcare needs. Nurses provide parent education, case management, developmental screenings, and ensure families are connected to medical and social resources within the community.
Individuals who have lost an infant unexpectedly
Sudden Infant Death Syndrome (SIDS) program provides grief and bereavement services to families and/or child care providers who have experienced the death of an infant due to SIDS or any Sudden Unexpected Infant Death (SUID). A Public Health Nurse will make contact with the family within three working days after notification from the coroner about the death, and provide supportive services, information, and referrals to community resources.
Community and provider education about SIDS are offered anytime.
Women, Infants and Children Supplemental Nutrition Program (WIC) serves infants and children up to age 5, pregnant women, and women who recently had a baby. Fathers, grandparents, foster parents of young children and working families are welcome to apply.

San Joaquin General Hospital general acute care facility providing a full range of inpatient services including General Medical/Surgical Care, High-Risk Obstetrics and Neonatal Intensive Care, Pediatrics and Intensive Care. In addition to the 196+-bed hospital, the medical campus includes multiple facilities dedicated to comprehensive outpatient services including Primary Care and Specialty Clinics. The Hospitals Ambulatory Care sites, including offices located at French Camp and Stockton, provide over 1,340,000 outpatient clinic visits a year.
In addition to providing direct medical services, the hospital is active in providing education for health professionals through post-graduate residency programs in General Surgery, Internal Medicine and Family Practice and has trained over 3,000 physicians since the residency programs were established in 1932. The hospital also participates in clinical affiliation agreements for training programs in a variety of health professions including Registered and Licensed Vocational nurses, Pharmacists, Radiology Technologies, Social Workers and Respiratory Therapists.
The Volunteer Office of San Joaquin General Hospital interviews and places volunteers in the hospital. Teens who are willing to make a six-month commitment are welcome to volunteer working the book cart, front information desk or gift shop. In addition, the Volunteer Office has introduced a new volunteer program, called "Las Teresitas" modeled after the teachings of Mother Teresa. The name Teresita is a befitting name for a program that, at its heart, contains compassion, love, respect and a sincere dedication for humanity. Healthy Beginnings provides pregnancy counseling and prenatal care to low-income women living in San Joaquin County. Classes and workshops are also available to teach infant CPR, general care of an infant, childbirth and parenting. Family planning counseling, emergency contraception and health maintenance of female reproductive organs are also provided. Healthy Beginnings accepts former and current alcohol and drug users. It refers high-risk patients to San Joaquin General Hospital.

San Joaquin Health Centers (SJ Health) Ambulatory Care Services provides primary care services for both children and adults. Three residency based clinics offers sub-specialty care management in geriatrics, OB/GYN and diabetes. Group classes and minor procedures are also provided. On-site retinal examinations are available daily.

St. Joseph’s Medical Center is a 359 bed acute care facility providing diagnostic and treatment services as well as emergency and critical care services. Hospital programs and services include physical therapy and outpatient surgery among others. Prenatal education classes are available through the Learning Center.

St. Josephs Cancer Institute (SJCI) is the only accredited cancer program in San Joaquin County. SJCI includes a dedicated inpatient oncology department and state-of-the-art radiation therapy.
In addition, St. Joseph’s provides cancer support groups and support for patients and families dealing with cancer. This includes a 6-week cancer survivng and thriving class, free medically-supervised exercise program to assist with fighting fatigue, gaining stamina, and more.
The Navigator Program is available to help anyone with cancer navigate through the journey of cancer from just receiving the news that the biopsy was positive, through the tests, procedures, treatment, follow-up, and long-term survivorship. St. Joseph’s Cancer Institute Support Group assists individuals in developing coping mechanisms. The group helps people create and use strategies on living with cancer. A person/family must be experiencing homelessness for all our programs other than the dining room for meals.
St. Marys Community Services exits to create pathways out of homelessness and poverty by offering essential daily services and supportive programs provided with respect and dignity.
St. Mary Community Services serves 3 meals a day, 7 days a week, 365 days of the year to members of the community in need of nourishing meals. People experiencing homelessness are able to access hot showers, clean clothing, social services, medical and dental care, recuperative care, womens lodge, mens lodge, and family lodge.
San Joaquin Homeless court is held on the last Friday of each month at 3:00 p.m. on the campus It allows individuals to clear up old traffic tickets, minor criminal charges and fines. Call the Resource Center for more information.

Stockton PACE is an integrated health plan and care team that provides medical and social services to keep the frailest elderly living in the community. An interdisciplinary team of doctors, nurses, social workers, rehab specialists, and aides are provided to meet the participants needs in the facility and in the community.

Stockton Pregnancy Control Medical Clinic provides elective abortions up to 14 weeks. The fees vary depending on length of pregnancy and if anesthesia is chosen. In addition, the RU-486 pill, the Plan B pill, sonograms and pregnancy tests are available. Now accepting new patients for Family Planning Services which include birth control and PAP smears. Testing for STIs are also available for females and males. Walk-ins are welcome.

Pacific Speech, Hearing & Language Center provides speech therapy services to adults in the community. Clients work one-on-one with student clinicians in the University of the Pacifics Department of Speech-Language Pathology. Therapy services are provided for fluency, speech, language and voice disorders, as well as cognitive rehabilitation following head trauma or stroke. In addition, aural rehabilitation for individuals with hearing impairment to help maximize communication effectiveness is offered.
The Department of Speech-Language Pathology also staffs the RiteCare Childhood Language Center, serving children from ages 2 to 18 years with speech and/or language disorders. Please call the Childrens clinic for more information on the program.
