Adult Day Services is a licensed program at Hutchins Street Square for older and disabled adults who cannot fully care for themselves. The program provides friendly, stimulating care and support through a variety of daily activities that encourage group participation. Caregiving families can also find support services through this program, as well as referrals to other programs and services available for older and disabled adults.

Support groups are available to help caregivers cope with the challenges of caring for an adult loved one.

American Cancer Society

American Cancer Society provides support to cancer patients and their families at no direct cost to patients or the people who care for them. This includes transportation to medical appointments, transportation reimbursement, lodging, support groups and wigs.

American Cancer Society also provides information on all types of cancer and prevention education. A live chat line is available.

Cancer Support Groups hosts two cancer support groups:

  • "Reach to Recovery" puts women with breast cancer into contact with other breast cancer survivors.
  • "Road to Recovery" coordinates a ride with an American Cancer Society volunteer driver. Coordinate a ride with a local organization to provide transportation, or refer you to a local resource that can help.

American Red Cross provides relief during a disaster to those in need of services. Blood Donation Centers serve as the sole provider of blood for 15 area hospitals. The Center draws, processes, stores, and delivers the blood. It also serves as a location for individuals to apply to join the National Marrow Donor Program.

Applied Behavior Consultants

Individuals with Autism Spectrum Disorders (ASD)

Fees: All services are funded by regional centers, school districts (non-public school/agency certifications), or insurance (co-payments may apply).

Applied Behavior Consultants (ABC), Inc. is an Applied Behavior Analysis (ABA) agency dedicated to providing the most up-to-date empirically sound interventions in the treatment of Autism Spectrum Disorders (ASD) and other related behavioral disorders/disabilities.

ABC offers the following services:

  • Center Based Services - This is a one-on-one ABA program for children between the ages of 18 months and up with a diagnosis of autism or ASD.
  • In-home Services - This program is for a child with autism or ASD between the ages of 24 months and 6 years of age. The overall goal is to prepare for integration into school.
  • Community-Based Services - ABC offers behavioral services to parents, Community Care Facilities (CCF), and schools.

Arthritis Foundation (Northern California and Northern Nevada)

California Poison Control System

California Poison Control System (CPCS) is the primary source of poison information and treatment advice for all California residents. Specially trained pharmacists, nurses, toxicologists, and health care providers quickly answer calls to 1-800-222-1222 and are available 24 hours a day, 7 days a week, 365 days a year.

All calls are free, confidential, and language interpreters are available in over 200 languages. Callers receive personalized treatment advice and/or a referral for medical care. Free health education materials can be ordered at www.calpoison.org.

Catholic Charities of the Diocese of Stockton (Health Care Access Program)

Eligibility:

Open to all San Joaquin County residents in need of health care.

Fees: None

Health Care Access Program assists uninsured individuals in applying for Medi-Cal, Medi-Cal Expansion and Kaiser Child Health Plan. Certified Enrollment Counselors (CECs) provide case management to clients to ensure utilization and retention. The program also assists with annual renewals and selection of a doctor or a dentist.

CECs help guide individuals with navigating the health care coverage system in both Stanislaus and San Joaquin Counties and are bilingual in Spanish/English. CECs provide outreach/presentations in the community and are knowledgeable about the Affordable Care Act.

Another service includes Healthy Smiles- connecting young children with Medi-Cal Dental services, assisting with identifying and selecting a dental home, and scheduling a dental appointment.

CDA Foundation (CDA Cares)

Eligibility:

Californians in need of dental care, including anyone having difficulty accessing dental care, uninsured, underinsured, unemployed, and adults and children of any age.

CDA Cares is to improve the oral health of all Californians by supporting the dental profession in its efforts to meet community needs. Oral health is critical to the total health and well-being of an individual. For young children, it means they won’t live with pain or miss school. For adults, it can enhance employment outcomes, improve their overall health and quality of life and, for many, restore dignity.

CDA Cares facilitates events and grant opportunities to community-based organizations and individuals who reduce barriers to care in underserved populations. The dental profession and its philanthropic values is represented as it generously gives back to help meet the needs of Californians. CDA Cares offers extractions, fillings, cleanings, oral health education and assistance finding a source for ongoing dental care. On a limited basis, CDA Cares also offers root canals, dentures and temporary partial dentures.

Doctors Hospital of Manteca (Growing Expectations)

Growing Expectations program is a hospital-based childbirth preparation class and a breastfeeding class that is offered to the community. A breastfeeding and maternal emotional support group is also available to all mothers regardless of the birth facility.

Please call (209) 239-8322 to inquire about the Childbirth Education class schedule and to register. The cost is $25.00. Classes take place from 9:00 a.m. to 4:00 p.m. on scheduled Saturdays. Esters Haven Adult Day Wellness Center

Must be at least 55-years old, ambulatory or semi-ambulatory without combative behavior.

Esters Haven Adult Day Wellness Service is designed to promote the physical, social, emotional, and cognitive needs of adults 55-years old and older. They provide a full range of facilitated and supervised therapeutic activities. Necessary respite is offered for families and caregivers that are caring for ambulatory individuals with chronic conditions like stroke, sensory limitations, Parkinsons, Alzheimers and other related dementia.

Gospel Center Rescue Mission (Recuperative Care)

Release from hospitalization by doctor or surgeon, homeless.

Recuperative Care by the Gospel Center Rescue Mission serves homeless transitioning from a hospital to the care that a family member would provide if the client were recuperating at home. GCRM operates the only non-profit Recuperative Care Program in San Joaquin County.

Each patients individualized plan of care is designed to fit specific needs and choices. Visits are as patient and family require or request by the appropriate hospice team member. A 24-hour on-call hospice nurse are available.

Respite care, support and practical help are offered. Counseling, spiritual support and grief support are also included. Some workshops have a one-time materials fee.

Hospice of San Joaquin offers services regardless of age, including pediatric hospice care. The agencys effective pain control and symptom management are tailored to the individuals changing conditions. Practical day-to-day support and guidance is given to family and caregivers as they adapt to changes in the health of their loves ones.

  • Hospice House, a comfortable "Home" when you need it most - is the first and only residential campus for hospice patients in San Joaquin County. This comfortable house offers an alternative to home based-hospice care when patients are no longer able to remain in their own home.
  • Transitions Program is a community service program designed to provide palliative education, emotional support and community resource information to participants with life-limiting or advancing chronic life-limiting illnesses.
  • The Bereavement Program supports grieving families and friends, including bereavement support groups for both adults and children. Hospice of San Joaquin also provides Community Education on end-of-life issues, and an intensive Volunteer Training course (up to 30 hours) preceding volunteer assignment to Patient Care or Bereavement Team. Health Unit offers free legal advice for health care consumers regardless of age or income. Assistance ranges from information and advice to representation at hearings and appeals. Advice is provided on all forms of coverage including through Covered California, Medi-Cal, Medicare, county programs, COBRA, employer group health plans, and others.

Lucile Packard Childrens Hospital (Stanford University Medical Center)

Northern Californian children referred by physician.

Stanford University Medical Center is the Northern California referral center for childrens catastrophic diseases, i.e. cystic fibrosis. Patients are ordinarily referred by their family physician. Low-cost or free lodging near this hospital is available for families of patients at the Ronald McDonald House.

The Lucile Packard Referral Center provides a listing of physician specialists for children. This information can be accessed by calling (800) 995-5724, Monday through Friday 8:00 a.m. to 5:00 p.m.

Mary Magdalene Community Services

Mary Magdalene Community Services offers services to adults, youths and children. Adult services include crisis intervention and referrals, transitional housing and case management, job skills, resume writing, and outreach. Services for youth and children include counseling, case management, and teen mentoring through the "My Brothers Keeper" program. Parenting classes and substance abuse classes are offered through court referrals.

Medi-Cal Access Program (MCAP)

Eligibility:

Must meet the following criteria:

  • Pregnant
  • A California resident
  • Ineligible for no-cost Medi-Cal or Medicare Part A & Part B
  • No maternity benefits through private insurance, unless deductible or co-payment is more than $500 (for maternity services)

Medi-Cal Access Program (MCAP) offers low-cost health coverage for mid-income pregnant women. It has been designed for middle income families without health insurance and whose income is too high for no-cost Medi-Cal. MCAP is also available to those who have health insurance if their deductible or co-payment for maternity services is more than $500. If you qualify for MCAP, your baby is automatically eligible for enrollment in the MCAP Infant Program unless your infant is enrolled in employer-sponsored insurance or no-cost Medi-Cal. Children grade K-6 whose family lives in a shelter within San Joaquin County and are without permanent housing or are in transition.

one.Charter Elementary is a kindergarten through sixth grade charter school. The school also serves children in transition or homeless children. The schools goal is to satisfy the physical, emotional and educational needs of children in San Joaquin County, using a strong academic program that includes standards-based education with a strong emphasis on literacy and math skills. Children are provided with breakfast, lunch, school supplies, clothing, and other necessities.

Counselors, interns and a school clinician are available to assist children through traumas associated with neglect, abandonment, physical, emotional, and sexual abuse as well as homelessness, hunger, physical and educational issues. The school also serves as a resource center for all homeless youth in San Joaquin County.

Bristol Hospice-Stockton works together with patients and their families to enhance quality of life. Hospice services are available for those typically affected by any type of life-limiting condition. By utilizing the most modern pain and symptom management techniques combined with a compassionate support system, hospice services help to reduce the overwhelming stress and anxiety that can come with caring for a loved one with a life-limiting illness at home or in a residency facility.

Hospice Care Team: When the focus of medical care changes from cure to comfort, the Hospice Care team provides physical, emotional, practical, and spiritual support for the entire family. Hospice provides home visits by nursing staff, medical social services, therapy services, physician services, hospice aide, and volunteer services.

Bereavement/Spiritual Counseling: Hospice also includes individual or group counseling. The professional team can provide individual counseling as well as offer regularly scheduled community groups. RiteCare Childhood Language Center of Stockton offers quality speech-language pathology services to children and individuals of the community, utilizing best practices, current research, and individualized client-centered treatment planning. Funding for the clinic facility comes from members of the Scottish Rite organization. There is an approximate eight month to a year wait to be evaluated. After the evaluation, the child will be placed on the waiting list for therapy, if warranted.

The clinicians at the Center are senior and graduate students working on a bachelor or master degree in Speech-Language Pathology at University of the Pacific, who are supervised by certified, licensed speech-language pathologists who are faculty members at University of the Pacific.

San Joaquin County Behavioral Health Services (Access Managed Care )

San Joaquin County residents.

Access Managed Care provides telephone linkage to services for individuals in need of mental health and/or substance use treatment. A clinical staff will speak to the caller or caregiver requesting services in order to determine need and eligibility. Upon completion of screening process, individuals can be referred to specialty mental health services, Non specialty mental health services, a substance use treatment program or their identified mental health provider through their health plan.

San Joaquin County Human Services Agency (Childrens and Adult Services)

While all of the programs are locally administered, most are federally and/or state-funded. San Joaquin County Human Services Agency (Medi-Cal)

Eligibility:

Individuals who are potentially eligible for Medi-Cal include those who are:

  • In a household with Modified Adjusted Gross Income (MAGI) below 138% of the Federal Poverty Level (FPL) for adults and 250% FPL for children.

Income levels are different for pregnant women and disabled or elderly populations. Individuals between the ages of 26 and 49 who are unable to provide proof of citizenship/legal resident status may be eligible for restricted Medi-Cal to cover pregnancy and emergency services.

Medi-Cal offers health care coverage for low-income residents of California who have little or no personal health insurance or are having trouble paying for medical services. San Joaquin County In-Home Supportive Services (IHSS) Public Authority (PA) (Registry Services)

Eligibility:

Recipients of Registry must be authorized for services by In-Home Supportive Services (IHSS). Individuals can apply to be listed on the Registry if they are interested in working as an IHSS home-care provider.

San Joaquin County In-Home Supportive Services (IHSS) Public Authority (PA) operates a Registry of available home care providers for IHSS recipients.

The IHSS PA offers training classes such as, infection control, fall prevention and dementia care and many others, via an online portal (Care Academy) and certificates are issued once a class is successfully completed.

The IHSS PA is always looking for individuals who are professional, courteous and responsible to work as IHSS providers through the IHSS PA Registry.

Hours are flexible and paid through the IHSS program.

San Joaquin County Public Health Services (Black Infant Health Program)

Eligibility:

Black and African-American pregnant and parenting women, age 16 years and older, and up to six months postpartum.

Fees: None

Black Infant Health is designed to encourage and support a healthy pregnancy, healthy newborn, and women’s health. Twenty group intervention sessions are provided (ten sessions prenatally, ten sessions postpartum), as well as complementary case management.

San Joaquin County Public Health Services (California Children’s Services (CCS))

Eligibility:

All four of the following must be met to be eligible:

  • Age - child/young adult must be under 21 years of age
  • Residence - child must be a resident of San Joaquin County
  • Income - family income must be less than $40,000 per year, according to the adjusted gross income on California income taxes. A child may be eligible when the family income is more that $40,000 if medical care for the childs CCS medical condition is expected to cost the family more than 20% of the familys yearly income.
    • There is no financial eligibility requirement for CCS diagnostic services or the CCS Medical Therapy Program.
  • Medical Conditions - Only certain medical conditions are eligible for CCS
Fees: Some fees for CCS clients who do not have Medi-Cal or who have Medi-Cal share of cost. The amount of the fees are based on family income and size.

California Children’s Services (CCS) is a statewide program that arranges, directs, and pays for medical care, equipment and rehabilitation when these services are authorized by the program. CCS pays for treatment, such as doctor services, hospital and surgical care, physical and/or occupational therapy, laboratory tests, X-rays, orthopedic appliances, and medical equipment as needed for the childs CCS-eligible condition.

Medical Therapy Program (MTP) is a special program within California Children’s Services that provides physical therapy, occupational therapy, and medical therapy conference services in outpatient clinics located in designated public schools for children who have handicapping conditions generally due to neurological or musculoskeletal disorders.

Services provided include:

  • “Medically necessary” physical therapy and occupational therapy services to eligible patients at no cost
  • Attendance at school meetings such an Individualized Education Plan (IEPs)
  • Home and school site visits to assess specific equipment needs
  • Evaluation and custom fitting of durable medical equipment, such as wheelchairs (if financially eligible)
  • Medical Therapy Conferences (Clinic) which are conducted by a team of health specialists who assist in managing your child’s therapy program and special needs. The conference is designed to provide a team approach to your child’s special needs at no cost.

Anyone, such as a parent, teacher, school nurse, or physician can refer a child or young adult to the MTP.

Therapy services are provided at rehabilitative units called "Medical Therapy Units" (MTU), in a school setting at four locations in San Joaquin County:

San Joaquin County Public Health Services Nurses Home Visiting program provides education on issues relating to pregnancy and infant development. Assistance is provided in obtaining prenatal care and health insurance coverage.

Home visits are provided to pregnant women, women with high risk pregnancies, high risk infants, postpartum mothers, and for infants and children with special healthcare needs. Nurses provide parent education, case management, developmental screenings, and ensure families are connected to medical and social resources within the community.

San Joaquin County Public Health Services (Sudden Infant Death Syndrome (SIDS))

Sudden Infant Death Syndrome (SIDS) program provides grief and bereavement services to families and/or child care providers who have experienced the death of an infant due to SIDS or any Sudden Unexpected Infant Death (SUID). A Public Health Nurse will make contact with the family within three working days after notification from the coroner about the death, and provide supportive services, information, and referrals to community resources.

Community and provider education about SIDS are offered anytime.

San Joaquin General Hospital general acute care facility providing a full range of inpatient services including General Medical/Surgical Care, High-Risk Obstetrics and Neonatal Intensive Care, Pediatrics and Intensive Care. In addition to the 196+-bed hospital, the medical campus includes multiple facilities dedicated to comprehensive outpatient services including Primary Care and Specialty Clinics. The Hospitals Ambulatory Care sites, including offices located at French Camp and Stockton, provide over 1,340,000 outpatient clinic visits a year.

In addition to providing direct medical services, the hospital is active in providing education for health professionals through post-graduate residency programs in General Surgery, Internal Medicine and Family Practice and has trained over 3,000 physicians since the residency programs were established in 1932. Healthy Beginnings provides pregnancy counseling and prenatal care to low-income women living in San Joaquin County. Classes and workshops are also available to teach infant CPR, general care of an infant, childbirth and parenting. Family planning counseling, emergency contraception and health maintenance of female reproductive organs are also provided. Healthy Beginnings accepts former and current alcohol and drug users. It refers high-risk patients to San Joaquin General Hospital.

San Joaquin Health Centers (SJ Health) Ambulatory Care Services provides primary care services for both children and adults. Three residency based clinics offers sub-specialty care management in geriatrics, OB/GYN and diabetes. Group classes and minor procedures are also provided. On-site retinal examinations are available daily.

San Joaquin Health Centers (SJ Health) also has a Family Practice Clinic in Stockton and in Manteca. The expansions are an effort to provide accessible and comprehensive healthcare for the whole family.

Shriners Hospitals for Children (Northern California)

Eligibility:

Children up to age 18 years who, in the opinion of their physicians, can benefit from specialized services available. Admission is based solely on a child’s medical need, regardless of finances.

Shriners Hospitals for Children is a one-of-a-kind international health care system of 22 hospitals dedicated to improving the lives of children by providing specialty pediatric care, innovative research and outstanding teaching programs.

Children up to the age of 18 years, with orthopedic conditions, burns, spinal cord injuries and cleft lip and palate are eligible for admission and receive all care in a family centered environment at no charge - regardless of financial need. Emergency assistance available.

St. Josephs Medical Center

St. Joseph’s Medical Center is a 359 bed acute care facility providing diagnostic and treatment services as well as emergency and critical care services. Hospital programs and services include physical therapy and outpatient surgery among others. Prenatal education classes are available through the Learning Center.

St. Josephs Medical Center (St Josephs Cancer Institute)

St. Josephs Cancer Institute (SJCI) is the only accredited cancer program in San Joaquin County. SJCI includes a dedicated inpatient oncology department and state-of-the-art radiation therapy.

In addition, St. Joseph’s provides cancer support groups and support for patients and families dealing with cancer. This includes a 6-week cancer survivng and thriving class, free medically-supervised exercise program to assist with fighting fatigue, gaining stamina, and more.

The Navigator Program is available to help anyone with cancer navigate through the journey of cancer from just receiving the news that the biopsy was positive, through the tests, procedures, treatment, follow-up, and long-term survivorship. St. Joseph’s Cancer Institute Support Group assists individuals in developing coping mechanisms. The group helps people create and use strategies on living with cancer. Discussion topics vary and include family relationships, stress, anxiety and other clinical and supportive information. A person/family must be experiencing homelessness for all our programs other than the dining room for meals.

St. Marys Community Services exits to create pathways out of homelessness and poverty by offering essential daily services and supportive programs provided with respect and dignity.

St. Mary Community Services serves 3 meals a day, 7 days a week, 365 days of the year to members of the community in need of nourishing meals. People experiencing homelessness are able to access hot showers, clean clothing, social services, medical and dental care, recuperative care, womens lodge, mens lodge, and family lodge.

San Joaquin Homeless court is held on the last Friday of each month at 3:00 p.m. on the campus It allows individuals to clear up old traffic tickets, minor criminal charges and fines. Call the Resource Center for more information.

Stellar Career College (Dental Assisting, Medical Assisting, Veterinary Assisting, Medical Office, Accounting Certificate Programs in Modesto)

Stockton PACE

Stockton PACE is an integrated health plan and care team that provides medical and social services to keep the frailest elderly living in the community. An interdisciplinary team of doctors, nurses, social workers, rehab specialists, and aides are provided to meet the participants needs in the facility and in the community.

Eligibility includes all of the following:

  • Be age 55 or older
  • Reside in San Joaquin or Stanislaus counties (zip codes at stocktonpace.welbehealth.com/about/)
  • Be certified by the State as eligible for nursing facility level of care
  • Be able to live safely at home and in the community without jeopardizing the health and safety of themselves and others

Stockton Pregnancy Control Medical Clinic

Fees: Vary; Medi-Cal accepted/Presumptive Medi-Cal.

Stockton Pregnancy Control Medical Clinic provides elective abortions up to 14 weeks. The fees vary depending on length of pregnancy and if anesthesia is chosen. In addition, the RU-486 pill, the Plan B pill, sonograms and pregnancy tests are available. Now accepting new patients for Family Planning Services which include birth control and PAP smears. Testing for STIs are also available for females and males. Walk-ins are welcome.

Stockton Scottish Rite Freemasonry

Stockton Scottish Rite of Freemasonry provides community services in the form of college scholarships and interest-free college loans. Scottish Rite also sponsors a clinic for childhood language disorders where no fees are charged, although voluntary donations from parents and the public are accepted.

Tracy Unified School District (Building Literacy Together)

Preschools enroll children living within Tracy Unified School District boundaries who turn four by December first and meet one or more of the high need criteria identified by First 5 of San Joaquin.

United States Department of Veterans Affairs (VA), Stockton Clinic, offers a health care center for military veterans. United States Department of Veterans Affairs operates the nations largest integrated health care system, with more than 1,700 hospitals, clinics, community living centers, domiciliaries, readjustment counseling centers, and other facilities. This clinic is part of VA Palo Alto Health Care System offering the following services: general internal medicine, a mental health clinic, smoking cessation, social work service, substance abuse and a wellness clinic.

Please note: no emergency services are available at this site.

University of the Pacific (Pacific Speech, Hearing & Language Center)

Eligibility:

Those who have a speech, hearing and/or language disorders.

Fees: No required fee; requested $200 donation per semester (12-weeks) to assist with purchasing of therapy materials.

Pacific Speech, Hearing & Language Center provides speech therapy services to adults in the community. Clients work one-on-one with student clinicians in the University of the Pacifics Department of Speech-Language Pathology. Therapy services are provided for fluency, speech, language and voice disorders, as well as cognitive rehabilitation following head trauma or stroke. In addition, aural rehabilitation for individuals with hearing impairment to help maximize communication effectiveness is offered.

The Department of Speech-Language Pathology also staffs the RiteCare Childhood Language Center, serving children from ages 2 to 18 years with speech and/or language disorders. Please call the Childrens clinic for more information on the program.

WestCare California (CalAim ECM and Community Supports Services)

Eligibility:

Those with an active Medi-Cal plan with Health Plan of San Joaquin.

Fees: No cost with active Medi-Cal plan (Health Plan of San Joaquin)

Enhanced Care Management (ECM) and Community Supports Services are focusing on breaking down the traditional walls of health care, extending beyond hospitals and health care settings into communities and providing high-need members with in-person care management.

Services provided:

  • Access to a single Lead Care Manager who provides comprehensive care management and coordinates health and health-related care and services.
  • Connections to quality care — at the doctor, the dentist, with a social worker, or at a community center.
  • Community Supports are services that help address members’ health-related social needs, help live healthier lives, and avoid higher, costlier levels of care. These include:
    • Support to secure and maintain housing.
    • Access to medically tailored meals to support short term recovery.
These services are available to eligible Medi-Cal members regardless of whether they qualify for ECM services. All Medi-Cal managed care plans (MCPs) are encouraged to offer as many of the 14 pre-approved Community Supports as possible.
